Exception logs that survive committee scrutiny
Structure exception and waiver records so a risk committee can see pattern, ageing, and policy intent — not just case volume.
Exception logs often show volume without meaning. Committees need to see which policy clause was bent, who approved it, how long it stayed open, and whether the same customer or product keeps returning.
A workable log captures the parent policy reference, the business reason in one sentence, the expiry date, and the compensating control. Sampling then becomes straightforward: overdue items, repeat requesters, and approvals below the stated seniority.
When we assess waiver programmes, the strongest firms treat the log as an early-warning system, not a filing cabinet. Patterns feed policy redesign; one-offs stay one-offs. That is what effectiveness looks like in practice.
